Be sure this form is complete, signed by the applicant, and signed by either two witnesses or by a notary. Attach to the completed form one (1) front and back copy of your valid photo identification card and one (1) completed FBI-Application Fingerprint card of your fingerprints. Fingerprints can be taken at local Sheriff’s Offices or police departments.

*Note: all original Applications must be printed and mailed with all necessary attachments to the State Banking Department. An original application must be completed for each office location that conducts business in Alabama pursuant to each act.
